About the role
Career Opportunity – Care Manager with Found Village in Cincinnati, OH Are you a collaborative, system-minded professional who believes every young person deserves a connected village working toward their future? Do you build trust across organizations, bring clarity to complexity, and anticipate needs before they become barriers? If so, we want to speak with you! The Care Manager brings together the people, resources, and opportunities surrounding each young person to create one connected support team. Working alongside Relationship Coaches and Navigators, this role aligns caregivers, schools, providers, employers, community organizations, and natural supports around shared developmental goals, clear responsibilities, and coordinated plans. While Relationship Coaches focus on developing young people, the Care Manager strengthens the village around them. More than coordinating services, this role connects people, plans, and systems so partners work together rather than independently. This is a nonclinical position focused on collaborative planning, care coordination, and follow-through—not counseling or clinical treatment. Salary range: $75k - $85k; depending on knowledge and experience . Benefits: Competitive pay and 401(k) match. Medical, dental, vision, disability, life, and other health-related benefit options. Generous PTO, sick leave, holidays, and built-in self-care days. Professional development funds and discounted YMCA membership. Responsibilities: Build trusted partnerships with young people, Relationship Coaches, Navigators, caregivers, schools, providers, employers, community partners, and natural supports across healthcare, education, child welfare, employment, housing, behavioral health, and transportation. Lead and refine shared developmental plans aligned with the Found Village Developmental Framework, clarifying goals, priorities, barriers, roles, next steps, and measures of success while strengthening youth participation, ownership, self-advocacy, and leadership in conversations about their future. Establish proactive communication rhythms, facilitate conversations that create clarity and shared accountability, and drive and stakeholder follow-through so partners understand how their roles reinforce coaching and advance youth development. Coordinate services, opportunities, resources, and transitions into one connected developmental journey, anticipating barriers and mobilizing support before challenges become crises, without creating unnecessary bureaucracy. Synthesize information from documentation, outcomes, data, and youth and stakeholder feedback to recognize patterns, identify barriers, assess progress, and translate findings into coordinated action and improvements to organizational systems. Requirements: Master’s degree in social work, counseling, or a related field preferred, with an understanding that this is a nonclinical planning and coordination role. Three or more years of demonstrated experience in collaborative planning, stakeholder coordination, facilitation, and complex service systems. Ability to coordinate stakeholders and build partnerships without formal authority. A systems minded approach to aligning the relationships, services, and resources around each young person. Analytical skills to synthesize information, identify patterns and barriers, assess progress, and guide coordinated action. Strong communication and facilitation skills, including navigating difficult conversations. Commitment to youth-centered planning and decision-making. Ability to balance empathy and accountability with calm, clarity, and follow-through. Strong organization, responsiveness, documentation, and continuous improvement. Initiative to anticipate needs, reduce service gaps, and support smooth transitions. Belief that coordination is care—not a burden young people should carry. Sound judgment and decision-making amid ambiguity. Experience in care planning and relationship-building, and the ability to direct coordinated action. Ability to lead and collaborate across teams as needs and circumstances change. What success looks like: Help young people shape their own plans and take growing ownership of their goals. Needs are anticipated and resources are in place before gaps become crises. The Care Manager partners across the village work together around shared goals, building alignment through trust and influence rather than formal authority. Why Found Village? Found Village believes every young person has strengths, potential, and a future worth investing in. The organization partners with families, schools, and community systems to surround teens with trusted, lasting relationships. The goal is not for young people to depend on Found Village forever. It is for them to leave Found Village with greater confidence, stronger connections, and deeper belonging within a caring community that continues to walk alongside them. Our values: Boundless Hope: Every young person has strengths, potential, and a future worth investing in. Courageous Love: We build strong relationships to encourage, challenge, confront, and remain present through difficult moments. Safe Place, Safe People: We create emotionally and physically safe relationships built on trust, dignity, and belonging. Collaborative Community: Young people are supported by an entire community, not just Found Village. Spend It Well: We invest time where it creates lasting developmental impact and strengthens relationships that endure. Apply online today! #ZR
