Practice Transformation Specialist, Florida Panhandle

Practice Transformation Specialists are HQ’s most direct interface with providers and can glean valuable insight into our partner practice’s happiness and needs. A successful PTS is passionate about population health, and highly mission oriented around providing better care to patients while reducing costs. At Aledade, a PTS can impact the health of their community by transforming one practice at a time. Note on Travel: This is a primarily remote role, but on-site check-ins are key. You can expect to travel in-person approximately 1x per month per practice (expected in the Florida panhandle). Exact schedules flex based on practice needs, meaning some months may be lighter or heavier than others.
